JUC's year-long Coming of Age program is the capstone of our middle-years’ program. In COA, 8th graders work with mentors, ministers, and facilitators to explore and define their spiritual (or not) beliefs and values. This experience will be something they can reflect on throughout their lives as they continue to transform and grow spiritually and ethically.

I give permission for the above-named minor child(ren) (hereinafter "Minor") to be photographed and/or video recorded by Jefferson Unitarian Church (JUC).
I understand and agree JUC may use the photographs and/or video recordings containing the words, image, and/or voice of the Minor in the recorded worship service, special church communications, or functions at Jefferson Unitarian Church and may be shared publicly through our website or social media. JUC will not use children's names in any images or recordings to protect your family and child's privacy unless separate written permission is given.
Parent, guardian, or legal representative of the above-named Minor can revoke release at any time by contacting Laura Lizut at laura@jeffersonunitarian.org.
